- This application must be submitted no later than ONE YEAR from the day of delivery.
- The following original certificates must be submitted together with the application:
(a) Certificate of birth of the child / children.
(b) A marriage certificate, unless such a certificate has been submitted in advance.
Original Certificate from the Banking / Collaborative Credit Institution, showing the beneficiary and the International Account Number (IBAN).
